Wellness Center Memberships

Individuals
3 month--$134.72 (must be paid in full)
6 month—$242.68/half year OR
               $40.45/mo.
12 month—$417.94/year OR
                 $34.83/mo.
($25.00 processing fee on 6 & 12 mo. memberships if NOT paid if full)

Senior Individuals (must be 62 or older)
6 month—$202.23/half year OR
                 $37.71/mo.
12 month—$350.53 OR
                 $29.21/mo.
($25.00 processing fee if NOT paid in full)

Student (must have school ID)
3 months - $105.93
(No processing fee!)

Household Discount:
If two individuals from the same household join on the same day, each membership will receive a discount and the processing fee is waived.       

 6 month membership - $15 off each            12 month membership - $40 off each

Dependent children, until the summer of College graduation, are FREE as long as there are two Individual Memberships in the same household.

*Memberships may be paid in full or by monthly EFT (electronic fund transfers) from your checking or savings account. NO processing fee when memberships paid in full.

Loyalty Rewards Program

As our thank-you for being loyal to Team Rehab, we are offering a Loyalty Rewards Program.  Here’s how it works…

New Loyalty Program (as of 12/1/09)
12 month members who renew their 12 month membership will receive an additional month for free. With every 12 month renewal thereafter, the member will receive one free additional month. To be eligible to participate, the member must always:

• renew for 12 months
• have no lapses in membership contract
• renew by your renewal date

Past Loyalty Program Members
(10%, 15%, 20% Off)
If your membership is currently in the 10%, 15% or 20% off Loyalty Program, you will remain in this same program as long as you renew for 12 months, have no lapses in your membership contract, and renew by your renewal date.  You are NOT eligible for the new Loyalty Program.
